Shows the sampling process and how participants were recruited in the community at different districts. It presents the sampling process from the district to the household level. Four districts out of the total 15 districts are presented in the left side of the graph. Three sub-districts from each of the four districts are presented in the graph that makes total twelve sub-districts. Individual participants were interviewed at the household level in each sub-district and presented in the right side of the graph. The sampling process is described in greater detail in the main text
